ControlTrack is a healthcare patient medication adherence platform that enhances the provider’s ability to monitor patients and includes three applications: Provider App, Pharmacy App and Patient App.

Provider App

When the provider enrolls a patient into ControlTrack, they create a personalized, de-identified patient profile that documents prescription adherence criteria and links uniquely serialized blister packs for pharmacy use.

ControlTrack monitors patient’s drug utilization using patient-captured, time stamped photos of their blister pack after removing a tablet for each dose.

ControlTrack has telehealth capabilities that enable providers to conduct the dosage checks virtually, without requiring the patient to visit the practice, which is particularly important relative to social distancing as well as address some of the challenges of rural medicine.

Pharmacy App

Patients enrolled in ControlTrack provide dispensing pharmacies their de-identified patient ID to verify the patient prescription information.

After sealing medication in the blister pack, the pharmacy will affix a uniquely serialized ControlTrack label, scan it, link it to the patient in the cloud and then dispense.

When dosages are adjusted, ControlTrack enables pharmacists to document how the remaining tablets against system expectations. Results are documented in the patient profile to assist the provider in reducing prescription misuse.

Patient App

A patient removes the medication from the blister pack and captures an image of it using the ControlTrack App. ControlTrack uses the barcode to provide prescription compliance details to the provider.

The Patient App also tracks and informs the patient how long it has been since they documented their last dose and how many doses should be remaining. This vastly improves the patient’s ability to manage their medications and identify when individuals of trust are taking their medications without their consent.
